dc.description.abstract PT.X is one of the manufacturing industry engaged in manufacturing & engineering of heavy equipment. At the first time PT.X was established, the company has an EHS (Environment Health and Safety) division. Nevertheless, there is still occupational accidents. In fact there have been 83 occupational accidents in PT.X from 2015 to 2017, which is about 85% percent more of occupational accident types are from production processes located in Plant 1. Lack of supervision and training provided by the company is the biggest factor causing the occurrence of occupational accidents, then the occurrence possibility of the accidents to happen again is very high. In fact from the last three years, the frequency of accidents has decreased, but the compensation cost to be incurred by the company has increased almost 2 times in each year. This occupational accident causes by increasing number of accidents that causes operators to lose working time (Lost Time Injury). Therefore, the purpose of this research is to analyze the handling priority of occupational accident that happened in PT.X especially at Plant 1 by using FMEA and FTA. Based on the results there are 6 types of Failure Modes priority with the highest RPN values, the sequence of handling priority are : Cutting and Welding by using oxy acetylene (384), Cutting Process by Using Plasma and Shearing Machine (336), Radial and Horizontal Drilling Process (280), Material Handling With Overhead Crane (252), and Rollbending and Bandsaw Process (210) en_US
