dc.description.abstract This research project focuses on minimizing the failures or risks that can occur in the process of supply chain process in PTTM. The failure in the supply chain process can cause great loses for the company as products cannot be supply to the customers resulting possible loss of orders. Having no proper planning and forecast for the import of goods to be distributed, lead time and other causes, the company often found themselves in the situation where there are orders but no goods to provide for their customer. FMEA will localize all the risks, probabilities, consequences estimated, and precautions taken to prevent the failure in the supply chain from happening. FMEA will provide the potential failure, cause of failure, and effect of the failure. Solution impacting to supply chain such as punctuality for delivery and cost reduction up to 3% from arranging the arrangement of delivery was resulted. en_US
